Player Check in will be at the Hazelrigg Student Center, 9th and Elm
from 12:30 -1:30 P.M. on June 7, 2012Player, Parent, Banquet & Program will be held Friday night @ 6:30 P.M. at the Masonic Center at 4th and Hastings
Shrine Classic Game Day Saturday June 9th 2012
Hastings College Girls 12:00 Noon. Boys 2:30 P.M.
IMPORTANT FORMS
PARENTAL CONSENT FORM Parental consent and proof of insurance are required before we can confirm your position on the team. This form will need to be mailed to us (address on Parental Consent Form) Also, send a wallet sized photo that we can use in our program.
PHYSICAL EXAM This is required by the Nebraska Activities Association & will be on file at your school. Please attach a copy along with your PARENT CONSENT FORM. If your school does not have this on file, we will accept an exam from your DR taken within the last 12 months.
CAMP SCHEDULE Players schedule for games.
ROOMATES Check the Roster for players you may want to room with and e-mail shrinesoccerclassic@gmail.com with the information. If you do not choose a roomate we will choose one for you.
GENERAL INFORMATION Please share with Parents.
