Parental consent and proof of insurance are required before we can confirm your position on the team. This form will need to be mailed to us (address on Parental Consent Form) Also, send a wallet sized photo that we can use in our program. This is required by the Nebraska
Activities Association & will be on file at your school. Please attach a copy along with your PARENT CONSENT FORM. If your school does not have this on file, we will accept an exam from your DR taken within the last 12 months. |
