WELCOME PARENTS!
Congratulations on your son or daughter being selected as one of the Players for the Annual Nebraska Shrine Soccer Classic Game to be held on Saturday June 9th 2012 at the Osborne Sports Complex, Hastings College, Hastings Nebraska.
In the Players Information Section, we asked your son or daughter to fill out several forms. If these forms have not yet been sent, please assist him or her in getting them completed A.S.A.P., we ask that this information be turned in by April 28th.
IMPORTANT FORMS
PARENTAL CONSENT FORM Parental consent and proof of insurance are required before we can confirm your position on the team. This form will need to be mailed to us (address on Parental Consent Form) Also, send a wallet sized photo that we can use in our program.
PHYSICAL EXAM This is required by the Nebraska Activities Association & will be on file at your school. Please attach a copy along with your PARENT CONSENT FORM. If your school does not have this on file, we will accept an exam from your DR taken within the last 12 months.
